Affiliation Forms College of Engineering, ISST Major To apply for affiliation with the Information Science, Systems, and Technology major in the College of Engineering, download the the affiliation forms [PDF] or pick them up in 303 Upson Hall. Please read the instructions carefully. In addition to the ISST affiliation forms, please include the appropriate College of Engineering form: either the Affiliation form, the Change of Major form, or the Add or Drop a Double Major form.
